Forms you will get:

• Request for Dismissal (Form CIV-110) – used when a tenant (or sometimes landlord) wants to dismiss the case
• Proof of Service (Form POS-030 or POS-040) – depending on whether the dismissal is served by mail or in person.
• Notice of Entry of Dismissal and Proof of Service (Form CIV-120) – confirms that the dismissal was entered and properly served.
